Community Support Service

Lewisham Community Support ServiceThe Lewisham Mental Health Community Support Service us commissioned by Lewisham CCG and Lewisham Council and is operating from July 2013. The service is particularly targeted for people suffering from mental health issues, aged 18 or over who live in Lewisham or are registered with a Lewisham GP. Individuals who want to use the service may be leaving community mental health services or just receiving support from their GP but need short term, practical and social support to improve their quality of life.

 

Individuals can access the service through:

* Short term (12 weeks up to 6 months) 1 to 1 support to manage mental health, live independently and achieve personal goals. This is delivered in people’s own homes or in community venues.

* Courses and workshops such as ‘Finance and benefits’, ‘coping with anxiety’, ‘managing mood’, ‘independent living skills’ and ‘coaching for change’.

* Information and advice on mental health, housing, finance, leisure and activities across the borough.

* Community bridge building, with information and support to access opportunities and services in the wider community.

* Focus groups which they involve service users who provide regular feedback to the service.

* Peer support and volunteering opportunities at Bromley and Lewisham Mind.

 

The community support service team consists of six mental health advisers and a service manager. Each adviser has a specific lead area:

* information and advice

* Advocacy

* BME

* Independent living

* Non-working age adults over 65 (x2 leads)

Individuals who want to access the service can self-refer. Referrals are also made by Community Mental Health Teams, GPs and voluntary and community organisations.
